Education, Training, Licenses, Certifications

• Bachelor’s degree.
• RN required, with current active RN license
• CCM certification preferred

Relevant Work Experience, Knowledge, Skills, and Abilities

• 4 – 6+ years of clinical experience.
• 5+ years of experience in care/case management, population health management, utilization review, quality assurance, or
discharge planning (preferably within a managed care, home care, or community health organization.
• Experience in case management/care coordination, managed care, and/or utilization management.
• Additional experience/specialized training may be considered in lieu of Bachelor’s degree.
• Experience with NYSDOH Medicaid program requirements and benefits under Long Term Care Proficiency in utilizing and
analyzing UAS-NY assessments and assessment outcomes.
• Organization/prioritization ability; and the ability to effectively manage a caseload of highly complex members.
• Experience with Long-term Services and Supports (LTSS), Managed Long Term Care (MLTC). .
• Strong communication skills (verbal, written, presentation, interpersonal).
• Computer proficiency: MS Office (Word, Excel, Powerpoint, Outlook); mobile technology (wireless phone/laptop, etc.).
• System user experience in a highly automated environment.
• Bilingual ability (verbal, written).
• Strong cross-group collaboration, teamwork, problem solving, and decision-making skills.
• Ability to work a flexible schedule (evenings, weekends and holidays) to meet member and/or caregiver and departmental
scheduling needs.

Pay Disclosure

A good faith estimate of the compensation range for individuals hired to work for the EmblemHealth Family of Companies is provided. This compensation range is specific and considers factors such as (but not limited to) the scope and responsibilities of the position, the candidate’s work experience, education/training, internal peer equity, and market and business considerations. The disclosed range estimate has not been adjusted for the applicable geographic differential associated with the location at which the work is being performed. It is not typical for an individual to be hired at or near the top of the range, as compensation decisions depend on each case’s facts and circumstances. Union roles covered by a collective bargaining agreement will compensate in accordance with the union contract.
