Care Manager, LTSS/HARP Programs (Remote requires NY Nursing License)
Qualifications:
- Bachelor’s degree
- Additional years of experience and/or specialized training may be considered in lieu of educational requirements (Required)
- RN required, with current active RN license for New York
- CCM certification (Preferred)
- 4 – 6 years of clinical experience (Required)
- Organization/prioritization ability; and the ability to effectively manage a caseload of highly complex members (Required)
- Support an integrated care model tapping into appropriate resources both internally and external to the organization (Required)
Experience in case management/care coordination, managed care, and/or utilization management (Preferred) - Strong communication skills (verbal, written, presentation, interpersonal) (Required)
- Trained in the use of Motivational Interviewing techniques (Preferred)
- Computer proficiency: MS Office (Word, Excel, Powerpoint, Outlook); mobile technology (wireless phone/laptop, etc.) (Required)
- System user experience in a highly automated environment (Required)
- Bilingual ability (verbal, written) (Preferred)
- Strong cross-group collaboration, teamwork, problem solving, and decision-making skills (Required)
- Ability to work a flexible schedule (evenings, weekends and holidays) to meet member and/or caregiver and departmentalscheduling needs (Required)

This compensation range is specific and considers factors such as (but not limited to) the scope and responsibilities of the position, the candidate’s work experience, education/training, internal peer equity, and market and business considerations. The disclosed range estimate has not been adjusted for the applicable geographic differential associated with the location at which the work is being performed. It is not typical for an individual to be hired at or near the top of the range, as compensation decisions depend on each case’s facts and circumstances. Union roles covered by a collective bargaining agreement will compensate in accordance with the union contract.
